Form of application for supply of electricity at Low Tension/ High Tension

Sir,

        I/We request you to supply electricity at Low Tension/ High Tension to my/ our premises situated at __________________________.

        The requisite information is furnished below :

  1. Name of the applicant (Consumer)

  2. Address for communication

  3. Status of the consumer (applicant should state the legal status i.e. whether he is an individual owner or lawful occupant of the premises for which the supply is required, partnership firm or limited company or any other local person ;and in case of partnership firm it should be stated whether it is registered or unregistered, location of its head office, names and addresses of all partners and a copy of the partnership deed should be filled. In case of a Company it should be stated whether it is a Private Ltd. Or Public Ltd., names of present Directors and a copy of the memorandum of articles of association should ;be furnished. In case of any other type of local person, full details and name and address of the person competent to represent ;the consumer should be stated).

  4. Location of the premises where supply is required.

  5. Character of supply.

  6. Purpose for which supply is required.

  7. Contracted load.

  8. Details of the connected load (The consumer should furnish the requirement for a period not less than 2 years and preferable for a period of 5 years).

  1. I/We hereby agree to take supply and to pay all charges such as for energy, service connection and other dues including deposit of such security as may be demanded in accordance with the rates and conditions of supply of the Board in force from time to time. I/We further declare and agree to take supply of electric power for the above-mentioned purpose for a period of not less ;than 2 years from the date of commencement of supply and, in case of re-connection of supply after a period of 6 months from the date of disconnection for not less than one year from the date of re-connection.

  2. I/We also undertake to receive supply within 3 months from the date the Board intimates that it is ready to give supply to our premises, failing which I/We undertake to pay the Board the minimum charges as may be applicable.

    2. Change of service connection in name of Shri ___________________ (In case of change of name, the particulars regarding the new consumer should be furnished in a separate sheet and attached to the application).

  1. I/We request that the Board should provide necessary meter/meters/metering equipments.

  2. The details of wiring carried out by a licensed wiring contractor shall be furnished after completion of the wiring. The Test Report shall be submitted as per the "Test Report" attached to the conditions of supply of the Meghalaya State Electricity Board.

  1. I/We shall undertake to execute an agreement in the prescribed form, if so called upon by the Board. Whether such an agreement was executive or not, this application itself shall oblige me/us to conform to and abide by the terms and conditions of supply in force/ notified by the Board from time to time.
